Relief of conditions and circumstances arising from illness

If you live in the Town of Evesham or the following designated parishes close to Evesham and are suffering chronic ill health, assistance may be available for help towards the cost of medication, medical aids or adaptations, equipment etc... for the relief of those conditions.

If you have a support worker such as physiotherapist, psychotherapist, occupational therapist etc.. and they are willing to support your request, please ask them to complete a Referral Agency Support Form which should be sent to us together with your Application Form. Both these forms can be obtained from the Downloads page.

The Charity is not able to supply equipment that is normally available via the NHS or which should be paid for from statutory funding, such as disabled living allowance or a disabled facilities grant. You or your support worker will need to show that statutory funding (if available) has been applied for and declined before applying to the Charity or that the required equipment is substantially different from that which is normally available.

Please provide your Doctor’s details when applying as a letter confirming your medical condition will be sought in respect of your application. Your consent will be requested before we contact your Doctor and you may request a copy of the response, if you so require.

Please note that all grants are financially assessed and are given at the sole discretion of the Trustees. Grant approval is also dependent upon funds being available within the agreed budget.

Approved grants are normally paid direct to the equipment supplier.
