Relief of conditions and circumstances arising from illness

If you live in the Town of Evesham or in one the following designated parishes close to Evesham and are suffering chronic ill health, assistance may be available for help towards the cost of medical aids or adaptations, equipment etc.. for the relief of those conditions.

If you have a support worker such as physiotherapist, occupational therapist etc.. and they are willing to support your request, please ask them to complete a Support Agency Referral Form as this will help us assess your request

The Charity is not able to supply equipment that is normally available via the NHS or which should be paid for from statutory funding, such as Disabled Living Allowance, Personal Independence Payment or a Disabled Facilities Grant. You or your support worker will need to show that statutory funding (if available) has been applied for and declined or unreasonably delayed before applying to the Charity or that the required equipment is substantially different from that which would normally be available.

Please provide your Doctor’s details when applying as a letter confirming your medical condition will be sought in respect of your application. Your consent will be requested before we contact your Doctor and you may request a copy of the response, if you so require. If your request relates to a mobility scooter or motorised wheelchair, an assessment of your capability to use the vehicle will be required. This is normally carried out at the Charity's cost.

Please note that all grant requests are financially assessed and are given at the sole discretion of the Trustees. Grant approval is also dependent upon funds being available within a specified budget.

Approved grants are normally paid direct to the equipment supplier.
